Who Should Complete the Form?

The form is intended for a student whose parent is declaring that the student has been under their exclusive and single parental care and custody.

The parent must provide their full name, National ID number, telephone number, relationship to the student and current marital status. The available marital status options on the form are Single, Separated and Divorce.

Does the Form Need to Be Witnessed?

Yes. The form includes a section for a Commissioner of Oaths/Magistrate.

The final section states that the applicant and their parent appeared before the officer and made a solemn declaration that the information provided is correct. It provides space for the signature, date and official stamp.
